I hereby attest that I have requested and authorized the Treating Doctor / Anbhu Ramachandran Speciality Hospital for an online consultation for me or for the person mentioned. The Treating Doctor will reach their conclusions regarding my diagnosis based solely on the information provided by me.

I understand and agree that:

Online consultation is solely based on the information provided by me.

• In the absence of a physical evaluation, the Treating Doctor may not be aware of certain facts that could limit or affect the diagnosis or recommended treatment.
• Online consultation differs from a regular face-to-face examination, and hence the diagnosis may be limited and provisional.
• An online consult is not intended to replace a full medical face-to-face evaluation by a doctor.
• The Treating Doctor assumes no responsibility for my continued medical care or treatment beyond the consultation.
My medical information will be handled with strict confidentiality, privacy, and security; however, I understand there are inherent risks with electronic transfers.

I assume the risks of these limitations and understand that no guarantee has been made regarding any particular result or outcome.

Communication Consent

I hereby completely and irrevocably release Anbhu Ramachandran Speciality Hospital, its doctors, consultants, healthcare professionals, staff, administrators, and directors (collectively, the “Hospital Parties”) from all claims, actions, or damages arising from or in connection with the online consultation, conclusions, or recommendations provided.

I agree that the Hospital Parties are not liable for the accuracy or completeness of the information provided by me or for any errors in electronic transmission.

By proceeding, I confirm that I have read this consent, understood its terms, and am providing it of my own free will.

Booking Appointments

While Anbhu Ramachandran Speciality Hospital will make every effort to ensure confirmed appointments, the hospital does not guarantee that all requests will be successfully scheduled.

If an appointment is confirmed but later canceled, postponed, or rescheduled by the doctor, the hospital will not be liable for any inconvenience caused.

Cancellations

Consultations (online or physical) can be booked, rescheduled, or canceled free of cost up to 60 minutes prior to the scheduled time.

You can reschedule an online consultation up to three times, and a physical consultation once.

Payment & Refund Policy

Payments may be made via UPI, Internet Banking, or Debit/Credit Cards using secure third-party payment gateways.

Once redirected to the payment gateway, you agree to their terms and conditions.

Refunds will only be processed for genuine cancellations made through the official Anbhu Ramachandran Hospital App or Website Dashboard.

Refunds for delays, dissatisfaction with advice, or health outcomes will not be entertained.

Refunds, if eligible, will be processed within 15 working days from the date of request.

All refund-related communication should be directed to the helpdesk.
